Genitourinary Syndrome of Menopause Treatment in Mumbai
Menopause is a natural transition, but the changes it brings—especially to intimate health—can significantly impact your comfort, confidence, and quality of life.
Genitourinary Syndrome of Menopause (GSM) is a common, chronic condition caused by declining estrogen levels, leading to changes in the labia, clitoris, vagina, urethra, and bladder.
If you are experiencing persistent vaginal dryness, burning, painful intercourse, or recurrent urinary issues, you are not alone, and these symptoms are highly treatable.
At our intimate medicine clinic in Mumbai, we understand the sensitive nature of GSM.

All You Need To Know About Genitourinary Syndrome of Menopause
What is Genitourinary Syndrome of Menopause (GSM)?
GSM is the updated term for what was previously known as Vulvovaginal Atrophy (VVA). It is a collection of signs and symptoms associated with the decline in estrogen and other sex steroids, primarily affecting the lower urinary tract and external/internal genitalia.
It is a progressive condition, meaning symptoms typically worsen over time if left untreated. Recognizing and treating GSM is vital for maintaining comfort, bladder health, and intimacy during and after menopause.
How the Treatments Work
Understanding the mechanism of action helps you choose the best treatment for your body.
- Local Hormonal Therapy (LHT):
- Mechanism: Direct estrogen replacement. The vaginal tissues contain estrogen receptors. LHT delivers low-dose estrogen directly to these receptors, essentially reversing the effects of menopause locally.
- Result: This causes the vaginal walls to become thicker (restoring tissue layers), more elastic, and increases the production of natural lubrication and acidity, which helps restore a healthy vaginal microbiome.
- Vaginal Laser or Radiofrequency (RF):
- Mechanism: Collagen and Neovascularization Stimulation. These devices use controlled thermal energy to create tiny, gentle micro-injuries in the sub-epithelial tissue (below the surface of the vagina).
- Result: This controlled injury triggers the body’s natural healing cascade, which involves producing new, high-quality collagen and elastin fibers, and growing new blood vessels (neovascularization). This physically rebuilds the tissue, making it thicker, firmer, and significantly increasing blood flow and lubrication naturally, without the use of hormones.
- Platelet-Rich Plasma (PRP):
- Mechanism: Bio-stimulation via Growth Factors. A small sample of the patient’s blood is processed to concentrate the platelets, which are rich in growth factors (proteins that stimulate healing and regeneration).
- Result: When injected into the vulvar or vaginal tissue, these growth factors activate local stem cells, promoting the repair of damaged tissue, increasing cellular regeneration, and enhancing blood flow. This leads to improved elasticity, lubrication, and sensation.
Treatment Techniques for GSM
We offer a personalized spectrum of treatments, ranging from conservative care to advanced regenerative procedures, tailored to your specific symptoms and health profile.
1. Non-Hormonal Options
These are typically the first line of defense for mild symptoms.
- Vaginal Moisturizers: Applied regularly (not just before sex) to hydrate the tissue.
- Personal Lubricants: Used during sexual activity to reduce friction and pain.
- Pelvic Floor Physical Therapy: Helps with pain, muscle tightness, and associated urinary issues.
2. Local Hormonal Therapy (LHT)
This involves low-dose estrogen delivery directly to the affected vaginal tissue, restoring thickness and moisture. Because the estrogen is absorbed locally, the systemic absorption is minimal, making it safe for most women.
- Vaginal Estrogen Creams: Applied directly inside the vagina.
- Vaginal Estrogen Tablets/Rings: Inserted periodically for sustained release.
3. Advanced Non-Hormonal Tissue Therapies (Lasers & Energy-Based Devices)
These are cutting-edge treatments that use targeted energy to stimulate the production of new collagen, increase blood flow, and rejuvenate the vaginal tissue, often without the need for hormones.
- Vaginal Laser or Radiofrequency (RF): A device gently inserted into the vagina delivers controlled energy, promoting natural healing and tissue thickening. This is highly effective for women who cannot or prefer not to use hormone therapy.
4. Regenerative Medicine
- Platelet-Rich Plasma (PRP): Using a patient’s own concentrated growth factors (harvested from a small blood sample), PRP is injected into specific areas of the vulva/vagina. This bio-stimulates cell growth, repairs tissue, and can improve lubrication and sensation, particularly in cases of severe atrophy or pain.
Who Is the Ideal Candidate for GSM Treatment?
Almost any woman experiencing symptoms related to GSM (dryness, pain, itching, urinary frequency) during or after menopause is a candidate for treatment. You are an ideal candidate if:
- You are post-menopausal and experiencing intimate discomfort.
- You find intercourse painful or impossible.
- You suffer from recurrent UTIs due to tissue changes.
- You have used lubricants/moisturizers with limited success.
- You are looking for non-hormonal solutions for your symptoms.
What to Expect During Your Consultation and Treatment
Your journey begins with a private and discreet consultation with our specialist.
- Thorough History: We will discuss your menopausal status, symptoms, medical history, and most importantly, your goals for treatment and intimacy.
- Gentle Examination: A compassionate physical assessment will be performed to confirm the diagnosis and assess the degree of atrophy.
- Personalized Plan: Based on the assessment, your preferences, and your health status, we will recommend a tailored treatment pathway (LHT, Laser, PRP, or a combination).
- Treatment Sessions: For energy-based treatments like laser, sessions are quick (15-20 minutes) and require little to no downtime. We ensure maximum comfort throughout the process.
Note: You can expect significant improvement in symptoms within a few weeks of starting hormonal therapy or after completing the initial laser/PRP sessions.
Potential Side Effects of GSM Treatments
While all treatments are generally safe and well-tolerated, it is important to be aware of potential side effects, which are usually mild and temporary.
- Non-Hormonal Options (Moisturizers/Lubricants):
- Rare irritation or allergic reaction to product ingredients.
- Lubricants containing glycerin or oils may increase the risk of fungal infections in some women.
- Local Hormonal Therapy (LHT):
- Mild, temporary vaginal discharge or spotting during initial use.
- Temporary breast tenderness or nausea (rare, due to minimal systemic absorption).
- Local irritation or itching at the application site.
- Vaginal Laser/Radiofrequency:
- Mild, temporary redness, swelling, or sensation of heat immediately after the procedure.
- A watery discharge for 1-3 days post-treatment.
- Mild, transient discomfort during the procedure.
- Platelet-Rich Plasma (PRP):
- Temporary swelling, bruising, or mild pain at the injection site.
- Minimal bleeding or soreness for 1-2 days.
- Since PRP uses your own blood, the risk of allergic reaction is virtually zero.
Important Questions to Ask Your Specialist
We encourage you to ask any questions you have. Here are a few key ones to consider:
- Based on my symptoms, is local hormonal therapy or laser treatment better for me?
- How quickly will I start to see relief from dryness and pain?
- Are there any activities (like swimming or exercise) I need to avoid after laser therapy?
- How often will I need follow-up treatments or maintenance?
- Will this treatment help reduce my urinary urgency/UTIs?
- How do you ensure my long-term comfort and sexual function?

Will Genitourinary Syndrome of Menopause (GSM) go away on its own?
Unfortunately, no. Unlike vasomotor symptoms (like hot flashes) which often diminish over time, GSM is a chronic and progressive condition caused by a permanent decline in estrogen. If left untreated, the symptoms of dryness, thinning tissue, and pain will likely persist and may worsen over the years. Treatment is necessary to manage symptoms and restore tissue health.
Is local vaginal estrogen therapy safe, especially if I have a history of cancer?
For the majority of women, low-dose local hormonal therapy (LHT) is considered very safe and remains the gold standard. Since the estrogen is delivered directly to the vaginal tissue, systemic absorption is minimal to negligible, leading to an extremely low risk of side effects compared to oral hormone replacement therapy (HRT). However, if you have a history of hormone-sensitive cancers (like certain types of breast cancer), a thorough discussion with your oncologist and our specialist is mandatory. For these cases, advanced non-hormonal options like Laser/Radiofrequency are often the preferred choice.
How does Vaginal Laser/Radiofrequency compare to Estrogen Cream?
Both are highly effective at relieving GSM symptoms.
- Local Hormonal Therapy (LHT): Works by chemically replacing the missing estrogen, which is absorbed by the tissue, making it thicker and more lubricated. It requires daily or weekly use.
- Laser/Radiofrequency (RF): Works by creating micro-injuries that stimulate the body’s natural healing response, promoting new collagen and blood vessel formation. This physically rebuilds the tissue. It is a series of quick, periodic in-office procedures and is an excellent option for women who cannot or prefer not to use hormones.
Does GSM affect my bladder health and lead to recurrent UTIs?
Yes, significantly. The estrogen receptors are concentrated in the lower urinary tract (urethra and bladder trigone). When estrogen declines, the urethral and bladder tissues thin and become less protective. Furthermore, the vaginal pH rises, which disrupts the natural, healthy flora (Lactobacilli) and makes the area more prone to bacterial overgrowth and recurrent Urinary Tract Infections (UTIs) and frequent, urgent urination. Treating the GSM directly with LHT or Laser can often reduce the incidence of UTIs and improve urinary urgency.
How long does it take to see results from GSM treatment?
elief depends on the chosen treatment:
- Moisturizers/Lubricants: Provide immediate, temporary comfort.
- Local Hormonal Therapy: Patients often start feeling noticeable relief from dryness and irritation within 2 to 4 weeks, with maximum benefits appearing after 3 months of consistent use.
- Laser/Radiofrequency: Improvement often begins after the second session, with full tissue remodeling and symptomatic relief achieved several weeks after the final session in the initial series.
